New Student Packet
Please fill out the following forms as soon as you are able. In order to register for fall semester courses, incoming students must complete and return these forms no later than May 22, 2026.
Required Forms
- Health Form
- Part I: Personal Health Data
- For student to fill out. Please submit within 2 weeks after completing the Intent to Enroll form.
- Part II: Physical Exam Results
- For healthcare provider to fill out.
- Part III: Immunization records, TB test result, & health insurance information
- Please note that students will not be allowed to enroll until all required health data are submitted and evaluated, including proof of insurance. Please complete all three parts of the Health Form (Part I, II, and III) by the stated deadline.
